Certificates, Training Records & Continuing Education

The document a participant receives depends on the course. Tampa Bay Training may issue its own certificate of completion, facilitate a credential administered by another organization or provide both.

Check the course page. A certificate of completion, certification, wallet card, operator evaluation, regulatory credential, PDH record and course attendance record are not interchangeable. The course page and applicable provider rules identify what is included.


Possible Certificate and Credential Issuers

Depending on the program, a certificate, card or completion record may be issued or administered by:

  • Tampa Bay Training;
  • A government agency or program administrator;
  • A curriculum or credentialing organization;
  • An OSHA Training Institute Education Center;
  • FEMA's Independent Study program;
  • HSI;
  • J. J. Keller;
  • CertifyUs; or
  • Another organization expressly identified on the course page.

Reference to an issuer identifies its role in the applicable program. It does not imply endorsement of every Tampa Bay Training course or business activity.


Requirements Before a Certificate Is Issued

A participant must satisfy the requirements applicable to the course, which may include:

  • Accurate registration and identity information;
  • Prerequisite documentation;
  • Full attendance and required instructional time;
  • Meaningful participation;
  • Required homework, activities or projects;
  • A passing examination or knowledge check;
  • A practical demonstration or operator evaluation;
  • Provider-specific registration or examination steps; and
  • An account in good standing.

Tampa Bay Training does not guarantee that every participant will pass or receive a credential.


OSHA Outreach Student Course Completion Cards

For OSHA 10-Hour and OSHA 30-Hour Outreach Training, Tampa Bay Training submits the required request through the applicable OSHA Training Institute Education Center after successful completion. The Education Center processes and sends the cards to the authorized trainer, and Tampa Bay Training then distributes them to the participant or client.

This process may take several weeks. An OSHA card is not available immediately at the end of class, and processing time is outside Tampa Bay Training's exclusive control.


HSI Digital and Physical Credentials

HSI credentials are normally issued digitally unless a different option is available and requested at registration. A request for a physical card must be made before the order deadline because cards and related materials may need to be purchased in advance.

A physical-card or replacement fee may apply. Confirm the available credential format on the applicable HSI course page.


FEMA Independent Study Certificates

For facilitated FEMA Independent Study review and test-preparation programs, FEMA controls the final examination and FEMA certificate. Tampa Bay Training helps participants prepare, provides local and practical context, assists with account setup and may provide supervised computer access.

Successful completion of Tampa Bay Training's additional instruction may also result in a separate Tampa Bay Training certificate when expressly stated. Tampa Bay Training cannot issue or guarantee a FEMA certificate.


Tampa Bay Training Certificates

A Tampa Bay Training certificate of completion documents successful completion of the requirements identified for the course. Unless the course page states otherwise, it is not:

  • A college degree or academic credit;
  • A professional license;
  • A government-issued certification;
  • An employer's authorization to perform work; or
  • A substitute for required site-specific training, supervision or evaluation.

Some Tampa Bay Training certificates do not expire. That does not prevent an employer, law, provider or industry practice from requiring refresher training.


Operator Training, Evaluation and Wallet Cards

An operator card should identify the equipment or evaluation actually completed. Classroom instruction alone does not establish full operator qualification when hands-on training, workplace evaluation or employer authorization is also required.

For forklift and aerial-lift programs, retraining or reevaluation may be required after unsafe operation, an accident or near miss, an identified deficiency, changed workplace conditions or introduction of different equipment. The course page explains any recurring evaluation policy or recommended cycle.


PDH and Continuing-Education Documentation

A regulatory course certificate does not automatically serve as a Professional Development Hour certificate. When Tampa Bay Training offers separate PDH documentation, the participant must request it and supply any information needed for the record.

The participant is responsible for confirming that a course and documentation satisfy the rules of the participant's licensing board, employer or other authority. Tampa Bay Training does not make licensing decisions for those organizations.


Certificate Delivery and Contact Information

Participants are responsible for monitoring the email account used at registration, including spam or junk folders. Notify Tampa Bay Training promptly if a name, email address or mailing address is incorrect.

An employer purchasing a private class may receive appropriate attendance and completion information for its participants. Tampa Bay Training will avoid disclosing unrelated personal or accommodation information.


Corrections, Replacements and Verification

To request a correction, replacement or verification, email service@tampabaytraining.com and provide:

  • Participant's name used during class;
  • Current name, if different;
  • Course title and approximate date;
  • Instructor or client organization, if known;
  • The document or correction requested; and
  • Information reasonably needed to verify identity.

Provider approval, processing time and replacement charges may apply. Tampa Bay Training cannot alter a record controlled by another organization, but we can help direct the request.


Record Retention

Tampa Bay Training retains course records for the period reasonably needed for training administration, credential verification, client contracts, provider requirements, accounting, legal obligations and dispute resolution. Different programs may require different retention periods.

Not every historical record can be guaranteed indefinitely. Participants and employers should retain their own certificates and renewal information.
